Boarding Release Form

In the event of an emergency, I understand that Metz Petz of Shawnee(hereafter, MPS) will attempt to contact me at the phone number(s) that I have provided; however, I understand that if I cannot be reached within a reasonable amount of time, I authorize Metz Petz Veterinary Clinic (hereafter, MPVC) to treat my pet as deemed necessary for its health and well-being and I agree to pay for any and all expenses that may be incurred.

I understand that if my pet is on medication I will provide the medications. I understand that if I do not provide enough medications or a special diet for the length of stay I will be charged for any replacement medication(s)/diet prescribed from MPVC Pharmacy. I understand I am required to list all medications at the time of drop-off.

I assume full financial responsibility for my animal(s). I understand that payment is expected when services are rendered. NO PET WILL BE RELEASED UNTIL THE BILL IS PAID IN FULL. I understand that a boarding environment is not without risk to my pet(s), neither MPS, MPVC, any employee, director, nor owner, will beheld liable in conjunction with any or all claims. Including but not limited to: disease, theft, fire, and injury, injury to persons, other pets or property by my pet.

We encourage you to bring your own food. However, if this is not possible, we will be feeding Dr. Gary’s Best Breed. If your pet(s) need a special prescription diet, you agree to let us know at drop-off.

We encourage you to bring and leave familiar articles for your pet during their stay with us. Please list items with the kennel attendant at drop-off.

If your pet has been given an appointment for any hospital/surgical procedures to be done during their stay, an additional release for treatment will be needed. Please let the reception/kennel staff know while filling out your boarding information.
